The Procedure

The Rhinoplasty Procedure: Open vs. Closed Technique

The rhinoplasty procedure usually takes around 1 to 3 hours. It begins with the administration of anesthesia — either general anesthesia or a combination of local anesthesia and sedation. Once the anesthesia takes effect, an incision is made in the target area. There are two general incision options:

Open Rhinoplasty: Creates a small incision across the columella (the external tissue between the nostrils), giving the surgeon maximum visibility and precision during reshaping. The scar is typically very small and discreet.

Closed Rhinoplasty: All incisions are made entirely within the nostrils, leaving no visible external scarring. This technique is ideal for patients requiring more limited modifications.

Dr. Aker recommends the approach best suited to each patient's anatomy and aesthetic goals. After the incisions are made, the surgeon modifies the bone, cartilage, and/or soft tissue to sculpt the nose into the desired shape. The incisions are then closed with sutures to complete the procedure.

After the procedure, the surgeon may place gauze in your nostrils to support the nose during initial healing, and you will be given prescriptions to make the healing process comfortable. What to expect:

  • First week: bruising and swelling are most noticeable, then gradually improve; avoid blowing your nose
  • Nasal tip: subtle swelling continues to resolve over 6–12 months
