Dr. Aker's endoscopic brow lift uses 3–5 small incisions (approximately 1–2 cm each) placed within the hairline. A miniature camera (endoscope) is inserted through one incision to visualize the tissues, while instruments are guided through the other incisions to release the brow attachments and elevate the forehead.

Once the brow is elevated to its target position, it is secured with internal fixation devices that dissolve over time after the tissue heals in place. The small incisions are closed with sutures or staples and are hidden within the hair. There is no long coronal incision running across the top of the scalp, and the hairline is not raised. The procedure is typically performed under general anesthesia and takes approximately 1–2 hours.
