What Is a Breast Lift (Mastopexy)?
A mastopexy — commonly called a breast lift — removes excess skin and reshapes breast tissue to lift and firm sagging breasts. During the procedure, Dr. Aker repositions the nipple-areola complex to a more youthful, forward-facing height and removes the stretched skin that causes the breast to sag. The result is a firmer, rounder breast profile with natural-looking contour.
A breast lift does not significantly change breast size. It restores shape and position using your existing tissue. For patients who also want more volume, a breast lift can be performed simultaneously with implants — a procedure called an augmentation mastopexy. Dr. Aker will help you determine during consultation whether a lift alone or a combined approach is right for your goals.
Breast ptosis (sagging) is a natural result of pregnancy, breastfeeding, significant weight loss, and aging. A mastopexy addresses what no bra, exercise program, or topical product can: the actual position and shape of the breast tissue.
Breast Lift Scar Patterns
The incision pattern Dr. Aker uses depends on the degree of breast ptosis (sagging). He will evaluate your anatomy and recommend the technique that achieves the best lift with the least necessary scarring.
- Crescent lift: minimal incision along the upper half of the areola border — for very mild ptosis only
- Periareolar (donut) lift: incision encircling the entire areola — suitable for mild ptosis; also allows areola size reduction
- Lollipop (vertical) lift: periareolar incision plus a vertical line from the areola to the breast fold — the most common technique for moderate ptosis
- Anchor (inverted-T) lift: lollipop plus a horizontal incision along the breast fold — used for significant ptosis or when combined with reduction
All breast lift scars soften and fade significantly over 12–18 months. Dr. Aker uses meticulous closure techniques to minimize scar visibility.
Who Is a Candidate for a Breast Lift?
Ideal candidates are in good general health, at or near a stable weight, and free of nicotine. If you are planning future pregnancies, Dr. Aker will discuss timing considerations — pregnancy after a lift can affect results. A mastopexy may be appropriate if you have one or more of the following:
- Sagging (ptotic) breasts — the nipple falls at or below the breast fold
- Downward-pointing nipples or nipples that point toward the floor
- Stretched or enlarged areolas
- Loss of breast shape and firmness after pregnancy and breastfeeding
- Changes in breast position following significant weight loss
- Loss of upper breast fullness that leaves the breast bottom-heavy
- Breasts that appear deflated when you are not wearing a bra

Recovery from a mastopexy is generally well tolerated. Here is what to expect:
- First week: rest and limited arm movement; a surgical bra is worn for support
- Return to work: most patients return to desk work within 1–2 weeks
- Bra transition: underwire bras may be worn after approximately 6 weeks, once cleared by Dr. Aker
- Exercise: light walking encouraged early; strenuous upper body activity restricted until 6 weeks post-op
- Swelling and bruising: gradual improvement over 4–8 weeks; final shape becomes visible as swelling resolves
- Full activity: cleared at the 6-week follow-up for most patients
